Replacement Certificate Request Form

This form is used to make a request for a replacement certificate issued by the New Zealand Institute of Health and Fitness (NZIHF). Please fill-out this form and the following payment process to initiate the request.

Before a certificate can be re-produced we will contact you for payment. Each Certificate costs $95 for a re-print. Payment in advance is required.
